Data Collection Form

The data collection form is the same as the one used by the Harvard-Hopkins team and is reprinted below.  It is necessary to fill one out before beginning the Study [the baseline data] and then provide one each month while participating in the Study.

It is imperative to complete the monthly form in order that TRI may properly monitor the Study progress.
